$3,000 Sign-on bonus for all full-time LPN and RN applications. Sign-on bonus will be paid out as follows:

You are hired for a full-time position and successfully complete a 30-day probationary period, you will receive a sign-on bonus of $300.
If you complete 60 days, you will receive a sign-on bonus of $300.
If you complete 90 days, you will receive a sign-on bonus of $400.
If you complete 120 days, you will receive a sign-on bonus of $400.
If you complete 150 days, you will receive a sign-on bonus of $400.
If you complete 180 days, you will receive an additional bonus of $400.
If the individual completes 270 days, you will receive an additional bonus of $400.
If the individual completes 1 year. you will receive an additional bonus of $400, for a total of $3,000.
(Employees who have given notice or terminated at any point during this period are ineligible for this incentive program.)
The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Provides direct patient care as defined in the state Nurse Practice Act.

  • Implements plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Clinical Supervisor.
  • Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the registered nurse.

Skills/Qualifications:

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, and organization skills.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

  • Possess and maintain valid CPR certification.
