Direct Support Professional – Ascentria Care Alliance – Portland, ME

Role Summary

Responsible for support of Individuals in their home and community. Responsible to Ascentria Care Alliance for supporting the Mission while conducting work activities within the context of policies and standards established for and by Ascentria Care Alliance.

Responsible for monitoring and role modeling health and safety for independent living, personal and social goals. To be an advocate and coach client rights while encouraging self-advocacy. Responsible for assisting individuals and encouraging growth and development of natural supports in their community. Responsible for coaching clients in development of skills to be independent and maintain their own home. Supports and facilitates team-based efforts with all members of the individuals’ team.

Essential Duties and Responsibilities

  • ensures individual plan is implemented
  • modeling appropriate communication and presentation in the community for individual
  • provide support and advocates for client as appropriate
  • assisting individuals to reach their goals/skill building
  • crisis and emergency behavior management
  • med administration and proper documentation compliance in accordance to State od ME regulatory guidelines and practices
  • work in partnership with individual to access activities/ community resources
  • ensure health and safety of individual
  • ensures nutritional/hygiene needs are met
  • provides assistance in personal care and medical needs as defined in individual plan
  • provide and model boundaries and limit setting
  • provides transportation as defined in plan and approved by Manager
  • understands what reporting is mandated
  • maintains and submits accurate and timely documentation to meet contractual/ licensing standards and policy and procedures.
  • teamwork – works well with others in the interest of the individuals we support
  • Employees are required to work any program that may have a need and will be assigned and directed to other programs prior to and or during scheduled shifts
  • attends and participates in mandatory team meetings and required trainings
  • assist in the evaluation and reporting requirements for PQI, including contributing to the development of outcomes, indicators and setting targets
  • participate in the collection and interpretation of data; and applying data to assist in improved practices for service delivery
  • other duties assigned and or associated with PQI to support the needs of the organization

Job Specifications

Education and Experience

High School Diploma or GED with no prior experience or training required.

Licensure, Registration and/or Certification

Valid State issued Drivers’ License and proof of current auto insurance required.

Obtain and maintain DSP/CDS, CPR/FA, behavior intervention training, CRMA and other trainings

mandated by Agency policies and State regulations.

Knowledge and Skills

Ability to make well-reasoned decisions, both independently and as part of a team.

Ability to follow-through to meet individual or agency needs.

Ability to use discretion in handling confidential information, compliance with HIPAA regulations.

Coping skills, such as flexibility in dealing with issues typical to a social service setting.

Ability to meet job performance measures with minimal direct supervision and guidance.

Language Skills

Ability to communicate clearly, in English, (speaking, reading, writing, listening and presentation with individuals

And service team members).

In our Deaf Services programs must have the ability to communicate using visual/ gestural communication a/o ASL.

Computer Operation skills

Proficient personal computer skills including electronic mail, word processing and internet.

Ability to learn applicable software required for documentation.

Working Conditions

Work is carried out indoors under desirable conditions with little or no disagreeable features and outdoors exposure to changing weather conditions. If guidelines are followed risks of exposure to hazardous conditions and blood borne pathogens is minimal.
