Hospital Clinical Liaison – Avamere Family of Companies – Portland, OR

The primary purpose of this position is to develop and maintain key relationships, generating referrals for admissions to drive census and clinical outcomes for the Avamere Family of Companies.

Responsibilities:

  • Act as a liaison between healthcare providers to ensure continuity of care for patients’ transition from hospital partners in the Portland metro area to our continuum of care environments, including nursing care facilities, assisted living and memory care facilities.
  • Serve as a resource for discharge planners, patients and/or their families, and educate them about our various services and programs.
  • Perform qualifying assessments to determine the appropriateness of all patient referrals.
  • Work with key physician groups at your assigned hospitals.
  • Deliver special presentations to groups of referral sources on services Avamere provides.
  • Participate in community organizations that directly impact referral sources and patient populations.
  • Expected to track and complete daily logs and monthly reports.

Qualifications/Requirements:

  • Must have a strong clinical background to be able to perform qualifying assessments of patients (eg. a licensed nurse, Respiratory Therapist, Physical Therapist, Occupational Therapist, Speech Pathologist, or MSW.) Non-licensed candidates with extensive admissions, case management or discharge planning experience will also be considered.
  • Bachelor’s degree preferred.
  • Must have 2 years of healthcare experience, ideally in a post-acute setting.
  • Must have previous experience with healthcare sales/outreach, or experience in a role where you were accomplishing measurable performance metrics.
  • This position requires working from our partner hospitals up to five days per week, rounding with case managers, patients, and families in hospital accounts.
  • Must have a valid driver’s license and dependable vehicle for transportation.
  • Must be able to travel overnight for mandatory meetings up to 5% of the year.
  • Able to work with many agencies and facilities, with exceptional attention to detail, communication and organization.
  • Must be self-motivated, self-managed and a self-learner; and able to be successful working independently.
  • Must be adaptable and flexible and thrive off a fast paced and ever-changing healthcare environment.
  • Able to navigate situations as “opportunities” rather than problems or issues.
  • Must possess clinical assessment and case management skills, with expert knowledge of various insurances and reimbursements.
