Certified Nurse Assistant CNA – Complete Home Health Care – Pueblo, CO

Job Summary: To provide skilled personal care and homemaking services as assigned to clients in their homes.


Reports To:
Clinical Supervisor, Branch Manager, Branch President


Responsibilities:

  • Is compliant with all policies, procedures, and regulations set by the Federal and State governments, the agency, and other regulatory agencies, i.e. OSHA, Worker’s Compensation, state licensure and labor laws.
  • Provides skilled personal care and necessary homemaking services to clients under the direction of the Case Manager and the established Home Health Aide Plan of Care
  • Completes and submits accurate documentation of visits in a timely manner as required.
  • Reports changes in client conditions to the Clinical Supervisor or Case Managing RN.
  • Abides by OSHA and agency rules for infection control and safety
  • Completes 12 in-service hours per year as required per licensure regulations.
  • Participates in after-hour, holiday, weekend, and on-call duties as requested.
  • Remains able to perform duties for clients whose attitudes and values may be different than the employee, while demonstrating a caring and compassionate attitude.
  • Maintains strict confidentiality of information relating to clients, employees, and the agency
  • Promotes customer satisfaction for all clients, family members, and visitors to the agency
  • Follows the agency’s Code of Ethics and Clients Rights
  • Approaches tasks with a positive and flexible attitude and practices team behaviors.
  • Performs other duties as assigned by the Clinical Supervisor, RN, Branch Manager, or President.
  • Maintains continuing education as required by state licensure standards and provides documentation of same.

Job Requirements:

  • Knowledge of CNA duties including personal care, skilled transfers, ROM, bowel programs and catheter care
  • Current CNA certificate from the State of Colorado
  • Must have worked as a CNA or received CNA training within the past 2 yrs
  • Current CPR training/card
  • Able to use blood pressure cuff, thermometer, stethoscope, personal protection equipment and gait belt correctly
  • Able to travel to client’s homes in all weather conditions
  • Able to manage time efficiently
  • Able to write or type progress notes legibly, read, and able to follow instructions
  • Able to promote strong interpersonal relationships and to deal with the demands of the position
  • Able to demonstrate effective oral and written communication skills
  • Able to lift up to 50 lbs unassisted or up to 325 lbs with an assistive device
  • Able to perform safely a stand/pivot transfer and a dependant transfer
  • Able to hear and observe client during treatment, as well as voice instructions clearly
  • Able to work in environments that may be unpredictable
  • Able to walk or navigate
