This position is full-time (40 hours/week) Monday – Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of (8:00am – 5:00pm).

As a Community Health Worker (CHW) , you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education.

Primary Responsibilities:

  • Create a positive experience and relationship with the member
  • Keep the member out of the hospital by supporting regular visits to their primary physician
  • Keep member actively engaged with their primary physician
  • Support the member to ensure pick – up of their Rx
  • Proactively engage the member to manage their care
  • Provide member education
  • Support transitions of care
  • Help to keep members compliant with their care plans
  • Partner with care team (community, providers, internal staff)
  • Knowledge and continued learning of community cultures and values
  • Conduct individual-level assessments to capture member’s current health care needs (e.g. Transitions of Care, Health Risk assessments)
  • Engage member to participate in the assessment process and collaboratively develop plan of care based on their individual needs, preferences, and objectives
  • Complete “Seek and engage” visits for members hat are unable to reach by phone

Required Qualifications:

  • High School Diploma / GED (or higher)
  • Must have resided within the local community for 2+ years
  • Must have knowledge of culture and values of community
  • Must have familiarity with the resources available in the community
  • Residing in a location that can receive a high-speed internet connection (additional information on UnitedHealth Group approved internet providers and restrictions will be provided)
  • Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
  • Ability to travel locally (up to 75%), up to 120 miles round trip (60 miles one way) (includes mileage reimbursement)
  • You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role
  • Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained.

Preferred Qualifications:

  • Bachelor’s Degree (or higher) in Social Work and / or Health Care Administration
  • Licensed Practical Nurse (LPN), Certified Nursing Assistant / Home Health Aide, Medical Assistant
  • CHW Accreditation
  • Experience working in Managed Care
  • Knowledge of Medicaid population
