· Must be physically able to lift, bend and stretch, climb stairs; Negotiate difficult pathways in and about the patient’s home.
· Support client/patient in turning, standing, sitting, transferring, and walking activities.
· Providing routine daily nursing care services, assisting under the supervision of the Registered Nurse (RN) reports to Director of Nursing (DON) or RN.
· Provides personal care services and performs services for clients as necessary to maintain their personal comfort and safety, Responsibilities include:
§ Transfers and positioning, grooming, assisting with meals, maintaining accurate documentation and other duties that may be assigned by the RN.
§ Follow simple procedures as an extension of therapy services under the direction and supervision of the certified therapist and RN.
§ Performs personal care activities, including: Bathing, Shampooing, Skin care/nail care, Oral hygiene, Shaving, Dressing and other duties as needed and assigned
§ Provide medication reminders only for clients who self-administer their medication
§ Performs household services essential to health care and safety in the home, including but not limited to: Meal preparation/feeding, Laundry and Light housekeeping
§ Reports any observed changes in the client’s physical or mental condition to the RN.
§ Complete and submit all agency required documentation as required, i.e. cares service notes.
§ Follow safe practices in the use of equipment; only attempt to use equipment fully trained to operate. Inform the DON or assigned RN when encounter unfamiliar process or equipment.
§ Report malfunctioning equipment to the DON and or RN.
§ Participation in staff and team meetings, as requested; and required in-service programs to meet compliance requirements.
§ Identify and report learning needs of the client, family or self to the DON or Administrator.
§ Performs other related duties and responsibilities as needed and assigned by the RN.
§ Keeps accurate visit records.
§ Recognizes the needs of the client and relates in a mature and caring way.
§ Present self in a neat, attractive, and professional manner consistent with dress code.
§ Organizes and manages time efficiently.
Demonstrates self-directed and responsible behavior.
Initiates activities to improve skills.
· Should demonstrate the ability to read, write, and follow a written plan of care.
· Must maintain current all Department certification requirements.
· Good interpersonal skills.
