Intervention Specialist – Sacramento Children’s Home – Sacramento, CA

OVERVIEW: The Intervention Specialist functions as the frontline worker for families who are experiencing a situation of instability, who feel overwhelmed and/or does not have the knowledge of resources when faced by challenges that divert the family’s focus away from effective parenting. This position is responsible for intervention and short-term community-based advocacy on behalf of families, including resource and referral services and actively participates in the multi-resource team.

DUTIES INCLUDE:

  • Performs assessment via phone or in-person to determine level of need, engages in intervention, and refers families to inter-agency programs or outside community resources based on the situation.

  • Provides intervention and short-term case management services, which connect families to services.

  • Works closely with various community partners and program components and establishes both formal and informal points of contact with area service providers to ensure a network of support for families.

  • Ensures families are connected to services.

  • Arranges for parents in crisis to utilize SCH Crisis Nursery for respite/day care when determined appropriate.

  • Refers and transitions families to effective parenting education workshops and services and provides support for assisting with attendance.
