Five Points Healthcare, a leader in skilled intermittent nursing and therapy services provided in patient’s homes, is seeking a qualified Clinical Manager to oversee and supervise all Hospice care for our patients and families in all settings.
The Clinical Manager will provide Hospice care to the terminally ill hospice patients. They will work as part of the Hospice team in providing care. They will oversee and supervise all Hospice care.
Essential Functions:
- Participates in developing standards which ensure safe and therapeutically effective service to patients and families. Has joint responsibility with the Administrator for seeing that standards are met. Participates in developing objectives for Hospice.
- Is responsible for recruiting, hiring, evaluating and terminating service personnel.
- Plans and arranges for consultation needs of staff; prepares and maintains current policies and procedures which meet Medicare, Medicaid, accreditation standards, state and federal laws, etc. and implements; revises concurrently.
- Organizes Hospice to delineate and delegate authority, functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service.
- Develops a cooperative relationship and communicates effectively and professionally with the physicians.
- Attends pertinent continuing education programs other than routine inservices and shares information with staff.
- Works with Administrator in identifying budgetary requirements and determining appropriate use of allocations.
- Ensures that patients’ plans of care are developed, implemented and evaluated.
- Ensures compliance with federal, state, and local rules, accreditation standards and Hospice policies in all patient care aspects of the Hospice.
Qualifications and Skills:
- Must have a minimum of 2 years experience as a Registered Nurse (RN)
- Graduate from an accredited school of nursing
- Currently licensed as an RN in the state of AL
- Active patient contact within the last 3 years preferred
- Ability to work independently, and to make accurate, and sometimes quick, judgments
- Ability and experience in supervising others
Job Type: Full-time
Pay: $46,601.00 – $73,156.00 per year
