Our ideal applicant will have Home Care experience.
The RN Case Manager will function as a part of a interdisciplinary team providing case management services to patients.
An RN Case Manager also will play the role of the patients advocate.
Job Requirements:
- Registered Nurse with Valid CT RN License
- 1 Year of RN experience; Homecare experience preferred
- Valid Driver’s License and reliable transportation
- Computer proficiency; preferred
