Broadstep, Inc. is hiring a Full Time LPN to provide assistance and leadership to our programs and staff in Milwaukee, WI.

JOB PURPOSE:

The LPN is responsible to ensure the premier therapeutic Nursing experience that meets and exceeds expectations both now and in the future through the implementation of each Consumer Experience Model (CEM) stage: (1) Pre-admission; (2) Admission; (3) Service Delivery; (4) Pre-discharge; and (5) Discharge. In the course of each consumer’s service delivery, the Registered Nurse ensures implementation of the prescribed Individual Service Plan in pursuit of the Desired Outcome of Treatment in a manner that communicates and instills Hope for their future, Safety at all times, Trust unconditionally, and Opportunity always.

The LPN facilitates these outcomes through each stage of the consumers’ Life Experience Model by (1) Knowing their Story; (2) Promoting Strengths; (3) Teaching and Learning, (4) Encouragement; and (5) Celebrating Success and engages each consumer in their life experience by providing opportunities for Living, Learning, Working, and Participating in the course of each consumer’s journey towards their Desired Outcome of Treatment.

The LPN serves as the liaison between each assigned consumer’s nursing, medical, psychiatric, and pharmaceutical needs and all internal and external healthcare resources necessary for the effective implementation the Broadstep, Inc. Consumer Experience Model. The LPN performs all communication and coordination with each consumer’s identified circle of supports throughout the consumer’s journey with our organization to include the following services: (1) Establishing and Maintaining a Nursing Relationship; (2) Desired Outcome of Nursing and Medical Treatment; (3) Nursing Assessment Services; (4) Nursing and Medical Service Plan Coordination; (5) Nursing Education; (6) Medication Administration Planning, Implementation, and Evaluation; (7) Crisis Management Services (Behavioral and Healthcare); (8) Medical Resource Coordination.

The LPN must be flexible to work a rotating on-call schedule with responsibilities to include evenings and weekends to meet the medical and cognitive behavioral needs of our individuals served.

Minimum Requirements:

Current WI LPN and two (2) years experience in a health care facility providing nursing services, (behavioral nursing preferred).

Must have a valid WI Drivers License.

Experience in therapeutic crisis intervention is desired. Must have strong clinical assessment skills.

Experience with Therap EMR system helpful.
