Social Service Company, located in Lakewood Ranch, FL
Candidate should hold a Master’s degree in Social Work (MSW), Gerontology or related degree OR be a Registered Nurse (RN) with experience working with the older adult population and/or their families.
Responsibilities include assessment, on-going oversight and management of the older adult, care plan development, assistance with placement, advocacy in senior communities and facilities, education and counseling. This position requires travel to the senior’s place of residence in Manatee and Sarasota County. Only qualified candidates please apply. This position has the possibility of being either 32 hours (4 days a week) or 40 hours per week (5 days a week.) Rate of pay is based on experience.
