Immediate full-time opportunity for a WellNess Nurse (LPN) at The Cohen Home. This is a beautiful, well maintained 42 bed community in Johns Creek. Must be able to handle med cart and available for every other weekend shifts. Great benefits and signing bonus. We conduct Covid 19 screenings daily!

Summary: The Wellness Nurse under the direction of the Director of Nursing is responsible for conducting resident care in the community to include the routine daily assessment and monthly wellness of residents, providing daily oversight of medication management, coordinating care with outside providers and updating care plans as needed in assisted living. The wellness nurse coordinates and communicate changes in care with residents and families and maintains regular and ongoing communication with families as needed. Partners with nursing management to provide insight to the wellness programs to include falls prevention, pain management, dietary and weight management, continence, and skin management, attends weekly at risk meetings, and assist in ensuring compliance with state and federal regulations.

Qualifications:

  • Must possess an unencumbered LPN license to practice nursing in the state of Georgia.
  • One (1) year of experience in assisted living, long term care or hospital industries or experience/exposure to the senior population
  • Must have or willing to complete CPR training
  • Ability to maintain a high level of confidentiality
  • Ability to handle multiple priorities
  • Knowledge of infection control practices and prevention of disease transmission
  • Knowledge of quality and clinical process improvement and risk assessment
  • Ability to delegate assignment to the appropriate individuals based on their skills, roles and interests
  • Possess written and verbal skills for effective communication and ability to facilitate small group presentations
  • Competent in organizational, time management skills
  • Demonstration of proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel) and Jewish Home Life Communities applications with the ability to learn new applications
  • Ability to work weekend, evenings and flexible hours, available for our customers at peak service delivery days and times.

Responsibilities:

  • Prioritize acute care needs of residents through assessment and reassessment to include identifying residents who require follow-up and have a change in condition.
  • Updates nursing management of all urgent resident care needs to include falls, transfers, returns/discharges to hospital in accordance with federal, state, local laws and Jewish Home Life Communities standards, guidelines and regulations.
  • Conduct resident rounding of residents involved in incident and require evaluation and follow-up, and communicate effectively with others from shift to shift and to others as necessary to facilitate the accuracy of any needed follow-up regarding resident care.
  • Ensures monthly documentation of resident wellness is completed and all communication to family members, residents, and responsible party is entered in the electronic health record.
  • Ensures that communication such as return phone calls, family grievances and concerns are addressed within 24 hours.
  • Notify management of the identified changes in condition and updates care plans as needed
  • Responsible for resident?s medical chart to include all required new move in paper-work is appropriately placed in the resident?s chart and notifying management of any missing documents
  • Completes resident assessments upon move-in, conducts monthly wellness visits, logs all incident reports to the electronic health record and follows up timely with all resident incidents.
  • Attends the community?s at risk management meeting.
  • Works with director of nursing to gather data to present at monthly Quality Assurance Performance (QAPI) Meeting.
  • Understands and adheres to the infection control programming of the community.
  • Serves as a liaison for ancillary health services to ensure physician orders are transcribed timely and prompt communication is conducted between team members and ancillary service providers.
  • Maintains log of incidents to include falls, antibiotics, skin tears, and hospitalizations and wound care.
  • Completes team member staffing and scheduling according to operational and budgetary guidelines.
  • Daily review of timekeeping practices and payroll reports to ensure all hours worked are recorded and
  • Maintains wellness files/resident charts according to JHLC and state and federal regulations
  • Inform the Director of Nursing of any medication and care level changes.
  • Utilize good judgement in seeking assistance from peers, supervisors, physicians, and other licensed providers, family, Responsible Party and EMS.
  • Obtains and maintains knowledge of community policies and procedures regarding all aspects of resident care, medication management, and infection control.
