The remote monitoring Nurse is responsible for monitoring, triaging, and engaging with patients on our VitalCare Care Platform on a daily basis. Our nurses provide excellent patient care via remote patient monitoring, telehealth, and social engagement software tools in between regular office visits to create a comprehensive remote care program designed to improve patient outcomes and improve overall patient satisfaction
Job Responsibilities:
· Provide exceptional virtual care for your panel of patients on a daily basis via text messaging, telephone or video calls.
· Provide continuity of care to patients with a wide range of acute conditions such as DM, HTN, COPD, CAD, etc.
· Collaborate with physicians on care team on complex patients and follow up with providers as necessary to close loops.
· Help develop clinical guidelines to continuously improve our care model.
· Work with Product and Operations to give feedback on ways to improve workflows to reflect best care practices.
Qualifications and Experience:
· Minimum 3 years of recent clinical experience as a Registered Nurse in areas such as ER/Urgent Care, Primary Care Setting, Ambulatory Care, Home Health, or ICU preferred.
· Multistate license under the Enhanced Nurse Licensure Compact (eNLC) and/or a New York, Conneticut nursing license are preferred. Active licensure in all 50 states would be an asset or the willingness to obtain licenses at the company’s request.
· Accredited nursing program graduate.
· Previous telephone triage experience using electronic triage software and computerized medical protocols considered is a plus.
· Strong clinical and strong communication skills.
· Exceptional computer skills within a Windows environment and keyboarding ability.
· Bilingual English/Spanish or another language will be considered is a plus.
