Responsible for resident services and other duties assigned, performing duties in conjunction with other staff and volunteers as appropriate. Duties include medication assistance/administration, nursing services, dining services and others as needed for resident well-being. Trains other staff as assigned.
- Adheres to and conveys philosophy of supporting dignity, privacy, independence, choice, individuality, and a home-like environment for residents.
- Acts as a lead trainer for staff, as instructed by Director of Nursing.
- Acts as on-call staff when requested by Director of Nursing.
- Provides back-up to Director of Nursing when needed. Receives and gives report from the previous shift.
- Reviews assignments with Director of Nursing and Administrator.
- Reads staff communication records, maintains familiarity with resident records, and documents in resident records as required.
- Evaluates health emergencies and determines emergency medical measures to be taken regarding resident care per LVN/LPN Scope of Practice and State Regulations.
- Assists with instrumental activities of daily living, ADL’s, environmental orientation, assistance or administration with medication, treatments and other care while encouraging self-care and independence, as permitted by state regulations.
- Provides intermittent nursing services within scope of license and state regulations.
- Assists with setting up, serving meals, and cleaning up afterwards, as requested. Provides services as indicated on resident treatment and service plans.
- Assists residents moving in and out, as assigned.• Assists with laundry service as requested
- Demonstrates knowledge of and follows infection control and safety procedures. Maintains clean, neat, comfortable, safe environment for residents, staff and visitors, including provision of housekeeping services for residents.
- Observes changes in resident status, needs or preferences, and communicates them appropriately to the Director of Nursing and/or Resident Care Manager.
- Observes other problems and communicates them appropriately.
- Demonstrates knowledge of and adheres to procedures for fire, life safety, disaster, security, work, safety, and other emergency procedures.
- Assists with individual and group social recreational activities.
- Notifies physician, RN and/or designated agent(s) of resident status/change as appropriate. Receives and executes written and verbal orders from the physician or designated agent, as permitted by state regulations. Performs other duties as assigned.
- Must possess a current LPN license. License must be in good standings.
- Must have a minimum 1 year of experience as a supervisor in a healthcare setting.
- Must have training in rehabilitative and restorative nursing practices.
- Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities.
