Full-time Board-Certified Nurse Practitioner to conduct Annual Wellness Exam In-Clinic or In-Home visits in Fort Myers, Florida
Position overview:
Conduct comprehensive In-Clinic Annual Wellness Exams on Medicare population with chronic disease conditions as well as assist in identifying and closing quality care gaps through labs and screenings. Educate patient on chronic health conditions, preventative screenings/tests and lifestyle concerns. Make recommendations and communicate findings to Primary Care Physicians.
- Board Certification with current unrestricted Nurse Practitioner (NP) License
- National Certification
- Willingness to travel
- Minimum 2 years’ experience preferred.
- Previous Annual Wellness visit or home health visits experience preferred
- Bilingual in English and Spanish preferred
- Previous EMR skills preferred
Job Types: Part-time, Contract
Pay: Up to $2,500.00 per week
