Assisted Living Facility Resident Case Manager – Fredonia Place of Williamsville – Williamsville, NY

Position title: Case Manager – Exempt (salary)

Supervisor: Administrator

General Summary:

Under the general supervision of the Administrator, the Case Manager is responsible for ensuring the provision of all necessary and prudent services to residents under 487.7 (g). S/he will oversee all record keeping practices and ensure they are effective and efficient. It is the Case Manager’s duty to assist residents and their families in adjusting successfully to the facility through provision of orientation, initiation into social activities, arranging interface with outside services, and referral as appropriate. Working relationships with the Assisted Living Coordinator, Memory Care Coordinator, Activities Director, Dietary Director, and Director of Maintenance is essential to meet and exceed the needs and services for the residents.

Education, Training, and Experience:

· A master’s degree in social work from an accredited college or university; or

· A bachelor’s degree from an accredited college or university with
major work in human services or service delivery and one year of fulltime experience in the provision of services to a frail adult population; or

· An associate degree from an accredited college or university with major work in human resources or service delivery and three years of full-time experience in the provision of services to a frail adult population.

Principal Duties and Responsibilities:

· Initial and periodic evaluation per policy, at least once every 12 months, of the needs of a resident and of the capability of the facility program to meet those needs;

· Orientating a new resident and family to the daily routine;

· Assisting each resident to adjust to life in the facility;

· Assisting each resident to maintain family and community ties and to develop new ones;

· Encouraging resident participation in facility and community activities;

· Establishing linkages with and arranging for services from public and private sources for income, health, mental health and social services;

· Assisting residents in making application for, and maintaining, income entitlements and public benefits;

· Assisting the resident in obtaining and maintaining a primary physician or source of medical care of choice, who is responsible for the overall management of the individual’s health and mental health needs;

· Assisting the resident in making arrangements to obtain services, examinations and reports needed to maintain or document the maintenance of the resident’s health or mental health, including:

· Health and mental health services;

· Dental services; and

· Medications;

· Providing information and referral;

· Coordinating the work of other case management and service providers within the facility;

· Assisting residents in need of alternative living arrangements to make and execute sound discharge or transfer plans; and

· Assisting in the establishment and operation of a system to enable residents to participate in planning for change or improvement in facility operations and programs and to present grievances and recommendations.

· Each resident shall be provided such case management services as are necessary to support the resident in maintaining independence of function and personal choice.

· Follow the system of recordkeeping, which documents the case management, needs of each resident and records case management activities undertaken to meet those needs.

· Shall utilize and cooperate with external service providers.

· Provide, without charge, space for residents to meet in privacy with service providers;

· Not inhibit access to individual residents who request services;

· Identify persons in need of services and assist external service providers in establishing a relationship with these residents;

· Work with service providers in executing a plan for service for individual residents; and
assist in arranging for transportation as necessary to ensure that residents are able to attend required services provided in an external location.

· Remain awake and alert for the entire working shift

· Follow all instructions provided in writing or verbally from any supervisor

· Serve as liaison between residents, families, physicians, and others as required

· Attend mandatory in-services

· Keep working areas clean and neat

· Consistently demonstrate respect, dignity and compassion for all residents

· Follow all Fredonia Place policies and procedures, including but not limited to the Department of Health regulations, Resident Rights, and Elder Abuse policies

· Conducting any evacuation or implementation of the disaster plan

Job Type: Full-time

Pay: $32,000.00 – $45,310.00 per year
