Discharge Planner/Case Management

The successful candidate will be responsible for collaborating with the medical team in identifying social needs of patients in the hospital. He/she will be responsible for performing needed assessments on patients and delivering Medicare Messages by staying up to date with CMS guidelines. The person in this role will utilize community resources to ensure that the patients have adequate follow up and care. Candidate will develop relationships with outside agencies and make referrals as appropriate. Successful candidate will work closely with the Utilization Management Nursing team and will share responsibilities, such as reviewing patients for medical necessity and coordination of patient care. Candidate will be able to coordinate and facilitate family meetings to discuss care options. Candidate will utilize the organization’s EMR to document discharge planning and social services in an ongoing manner. Candidate must demonstrate the ability to be flexible, organized and function under stressful situations. Candidate consults other departments as appropriate to collaborate on patient care and performance improvement activities. Some on-call hours are required. The successful candidate must be able to work well independently and within a multi-faceted team environment. The ability to relate well with multiple stakeholders including physicians, nurses, allied health professionals, outside agencies, and patients is a must.
