HIV Integrated Care Program Clinical Social Worker – SAC Health System – San Bernardino, CA

This position is eligible for possible loan repayment assistance through HRSA. SAC Health System is a Federally Qualified Health Care Center.

The HIV Prevention Integrated Program Clinic at SAC Health System recognizes the complex health care needs of the LGBTQ patient population. The HIV Prevention (PrEP) Integrated Program Clinic is a newly funded program with exciting opportunity for growth and continued funding.

The HIV PrEP Social Worker will act as an integral part of the program’s growth and will actively seek and recruit clients who are at higher risk for becoming infected by HIV; engage clients into integrated HIV Primary Care and Behavioral Health services, as needed.

The HIV PrEP Social Worker will provide patient support and navigation services to HIV negative individuals who are at higher risk for becoming infected by HIV and/or who may have barriers to accessing and adhering to PrEP care. The HIV PrEP Social Worker will work with Specialty Providers and collaborative partners to assure all HIV Prevention Care clients have optimal care and coordination of services, including outside referrals as appropriate. Services may include housing, income and food procurement; medical and other benefits enrollment; access to medication assistance, mental health counseling, and substance abuse treatment; and, outside specialty care, laboratory, pharmaceutical services, and other services to enable health maintenance.

The HIV PrEP Social Worker will provide traditional psychotherapy in the Health, Behavior, and Wellness Center, will participate as a Health and Wellness Consultant as part of the Integrated Care program providing assessment and brief intervention in the medical clinics as part of the multidisciplinary healthcare team, and will co-facilitate health education groups.

This position will include travel to other SACHS clinic sites as needed.

Experience:

Trauma-informed clinical orientation preferred.

Knowledge and experience working with at-risk communities for HIV and Hepatitis C.

Culturally responsive to ethnic minorities; HIV+/ at-risk people, including sex workers, substance users and LGBTQ community members.

Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.

Experience treating people living with behavioral and mental health challenges; familiarity and experience using counseling and therapeutic techniques, so that clinical and mental health services can be offered when needed.

Comfortable talking about safer sex practices and familiar with harm reduction best practices.

Maintain professional development in new prevention strategies, methodologies and resources available to HIV/AIDS or Hepatitis infected people.

Bi-lingual preferred.

Required Education:

Licensed Social Worker, Board of Behavioral Science License Required. Master’s Degree in Social Work.

1 year of experience in a community-based setting working in diverse settings with adults-LGBTQ preferred.

Must be able to work in the U.S. legally. We are unable to sponsor H1B Visa’s.
