Director of Care Management

Salary: $60k-70K; Fulltime – 40 hours; Monday – Friday from 9am – 5pm
Location: All boroughs, especially Queens and Brooklyn

Director of Care Management Requirements:

2 years supervising MSC/Care Management team
BA + 5 years’ experience in I/DD Care Management or master’s degree + 3 years’ experience in I/DD Care Management.
Absolute sense of integrity and personal commitment to serving people with I/DD and their families.
Bilingual candidates highly desired (Mandarin, Cantonese, Spanish)
Must be able to travel 50% of the week to meet with clients.

Care Manager Supervisors

Salary: $50-60ks; Fulltime – 40 hours; Monday – Friday from 9am – 5pm
Location: All boroughs, especially Queens and Brooklyn

Care Manager Supervisor Requirements:

1-year experience providing Care Management (MSC) + Master’s degree or 2 years’ experience providing Care Management /(MSC) + BA
Absolute sense of integrity and personal commitment to serving people with I/DD and their families.
Bilingual candidates highly desired (Mandarin, Cantonese, Spanish).
Must be able to travel 50% of the week to meet with clients.
Care Managers

Salary: $40ks; Fulltime – 40 hours; Monday – Friday from 9am – 5pm
Location: All boroughs, especially Queens and Brooklyn

Care Manager Requirements:

A Bachelor of Arts or Science degree with two years of relevant experience, OR a license as a Registered Nurse with two years of relevant experience, OR a Master’s degree with one year of relevant experience.
Absolute sense of integrity and personal commitment to serving people with I/DD and their families.
Bilingual candidates highly desired (Mandarin, Cantonese, Spanish).
Must be able to travel 50% of the week to meet with clients.
Bilingual Intake and Benefit Access Coordinator

Salary: $45k; Fulltime – 40 hours; Monday – Friday from 9am – 5pm
Location: All boroughs, especially Queens and Brooklyn

Bilingual Intake and Benefit Access Coordinator Requirements:

Bachelor’s degree plus one-year experience completing enrollment for OPWDD supports and services
Must have knowledge regarding access to and support to the maintenance of member benefits (Medicaid).
Absolute sense of integrity and personal commitment to serving people with I/DD and their families.
Must be bilingual in English as well as in Mandarin, Cantonese OR Spanish
Must be able to travel 50% of the week to meet with clients.
