Overview

The Ryan White Complex Care Case Manager is an instrumental position in the design and implementation of MCC’s new Ryan White case management program. The Ryan White Complex Care Manager provides ongoing case management for MCC patients living with HIV/AIDs and other complex medical conditions and psychosocial needs. The case manager provides comprehensive support to his/her identified panel of patients with the goals of minimizing barriers to care, improving health outcomes, and helping patients develop the ability to manage their needs with greater independence. Working collaboratively with primary care, behavioral health, nursing and members of the care navigator team, the complex care case manager works as a member of an integrated team of professionals to provide patient-centered, outcome driven care in the context of the MCC medical home.

Responsibilities

Plays key role in facilitating the development of MCC’s Ryan White Case management program;
Provide psychosocial assessment and clinical case management services to MCC patients with HIV/AIDs and other complex medical conditions and psychosocial needs;
Provide support & clinical services guidance to Ryan White care team and care navigators;
Maintain and track services for panel of identified patients as required by Ryan White Program and MCC;
Ability to complete psychosocial needs assessment and subsequent treatment plan;
Provide crisis intervention as indicated.
Provide consultation to medical providers regarding the psychosocial components of a patient’s presentation. Develop collaborative plans to address psychosocial needs.
Work with patients in their preferred language (primarily English and Spanish), using a language line as needed.
Use the “warm hand off” method (immediate brief assessment and intervention) for assisting patients who have identified psychosocial needs at the time of their medical visit.
Experience with or a willingness to acquire specific knowledge HIV/AIDs and other chronic health conditions;
Demonstrate a willingness to acquire general knowledge of psychopharmacology, and engage in ongoing learning about new medications and treatments commonly used for mental disorders, substance abuse, and general health problems.

Use the Clinic’s electronic health record (EHR) to document clinical work, produce billing, collect service data, and maintain a schedule, as well as to communicate information to other clinic providers.
Work closely with behavioral health, care navigation MAs, nursing and medical providers.
Attend behavioral health meetings, care navigation meetings, medical provider meetings, all-staff meetings, as scheduled. May be asked to participate in Ryan White County-level meetings.
Provide frank and considered opinions regarding case management services and quality improvement measures. May be asked to consult with junior care navigators about care navigation activities.
Provide services to satellite clinics as needed; may be asked to work variable schedule during extended-hour clinics (e.g., 10am-7pm on one or more days).
Must be able to work independently;
Other duties may be assigned.

Reports to: Manager of Complex Care Case Management

Qualifications

Licensed Clinical Social Worker (LCSW). The successful applicant will demonstrate a commitment to providing case management services to medically complex/vulnerable populations. Experience working with individuals living with HIV/AIDS and/or substance-using individuals highly preferable. Knowledge of state regulations and standards related to client treatment, patient rights, and client/patient confidentiality required. 1-2 years of clinical or behavioral health experience in a health care setting caring highly desirable. Experience with & commitment to motivational interviewing required. Commitment to cultural humility required. Demonstrated ability to work effectively on a team, show initiative, flexibility and enhance a professional culture of trust and good will. Experience in primary care environment preferred.

Language Skills:

Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence in English.

Bilingual in Spanish highly desired but not required.

Computer Skills:

To perform this job successfully, an individual must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.
