Pulmonary Hypertension Program Coordinator – Fresno California The Central California Faculty Medical Group (CCFMG) affiliated with UCSF Fresno Medical Education and Research Program has a position available for a full time licensed Nurse Practitioner to serve as a Program Coordinator to provide leadership, Nursing and patient education, direct assessment, evaluation and care to critically and/or chronically ill patients with Pulmonary Hypertension. It is a leadership position directly involved with the Community Regional Medical Center (CRMC) Pulmonary Hypertension Program development and evaluation. Ideal candidates will have a track record of expertise in pain and symptom management, complex communication, shared decision making, and collaborative provision of education to patients with a variety of life-limiting illnesses and their families. National Certification and CA licensure with 2 years’ relevant clinical experience, including prescribing is required.
