Case Manager – South Metro Human Services – Saint Paul, MN

Objective:
To provide professional, quality program services to the clients served by Targeted Case Management. Work with other members of the team to assist clients in obtaining and maintaining an optimum quality of life consistent with client needs and preferences. Emphasis is placed on working with clients in community settings and directly providing the services necessary to assist clients in achieving their goals.

Hours: Mon-Fri days

Qualifications:

  • Bachelors Degree in Behavioral Sciences or related field.
  • 2,000 hours supervised experience working with individuals with Mental Illness.

Duties and Responsibilities:

  • Develop and implement the treatment plan that is based on the diagnostic and functional assessment which is approved by the clinical supervisor.
  • Assume with other members of Team responsibility for assuring compliance with Minnesota Rule Chapter 9520. Provide or ensure access to needed and eligible medical, vocational, social, educational, financial, legal, and housing and chemical dependency services.
  • Develop an Individual Community Support Plan for each client every 6 months.
  • Schedule face to face visits as dictated by the ICSP.
  • Make appropriate / culturally sensitive referrals and assist clients in gaining access to needed services.
  • Participate in the commitment process as mandated: provide testimony in probate court, write all required court reports, monitor stayed warrants and provisional discharge agreements and assist pre-petition screening process as necessary.
  • File all legally required reports with Vulnerable Adults, the Ombudsman and Child Protection.
  • Assume with other members of team responsibility for providing the services necessary to maintain and enhance each client’s ability to live as independently as possible.
  • Develop an individualized emergency support plan coordinated with community emergency support services.
  • Assist client with housing issues and interface with HRA and PHA staff as well as landlords.
  • Attend scheduled quarterly reviews and treatment planning meetings.
  • Collaborate with, support and provide education to family members and others interested in the client’s treatment and welfare when warranted.
  • Provide education to clients regarding the symptoms, course and treatment of their illness.
  • Provide team and agency coverage of client needs.
  • Must be able to transport clients in the community.
  • Participate fully as a member of a team with an emphasis on generalist rather than specialist practice.
  • Develop appropriate therapeutic relationship with clients.
  • Attend all team meetings as scheduled.
  • Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
  • Maintain a pattern of regular work hours.
  • Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
  • Meet with supervisor as specified for clinical supervision, minimum of 19 hours of individual and 19 hours of group.
  • Maintain established community standard of care.
  • Other duties as assigned.

CEU Requirements:

  • 30 Continuing Education (CEU) Credits every 2 years.

Competencies:

  • Knowledge of symptoms and course of serious and persistent mental illness as well as medications and other therapies.
  • Have knowledge of evidence-based clinical practices.
  • Be knowledgeable about and utilize various substance abuse treatment modalities such as harm reduction and motivational interviewing.
  • Have demonstrable working knowledge of vulnerable adults laws and other pertinent rules and regulations.
  • Knowledge of supportive counseling techniques, independent living skills/teaching and crisis intervention techniques.
  • Knowledge of current principles, practices and techniques of case management.
  • Knowledge of community resources.
  • Able to assess and manage crisis situations.
  • Understanding of crisis assessment tools for suicidal and aggressive behavior.
  • Able to use a computer for word-processing and data base management.
  • Able to work independently and as a member of an interdisciplinary team.
  • Be able to speak, clearly understand and write the English language.
  • Must have a car, MN driver’s license and be able to drive, day and evening hours.
  • Be able to accommodate flexible working hours.
