The Team Manager (TM) is responsible for managing the day-by-day activities of an interdisciplinary team including supervision, resource management, scheduling and assignments. The TM ensures that all patient care is provided in accordance with established organizational procedures and government regulations. The TM aids in recruitment process, develops, supervises, and trains clinical staff. The TM also actively participates in on going process improvement.
Responsible for management of related disciplines to ensure coordination of care of hospice services at the point of admission. Responsible for facilitating communication and consultation with the hospice physician or Medical Director as needed regarding issues related to eligibility, plan of treatment or attending physician needs. Coordinates, develops and monitors activities related to intake, admissions process and data entry in collaboration with Quality Management department.
