La Casa Family Health Center has an immediate opening for a full-time Care Coordinator I. This is an administrative support position that works remotely and may be required to work on site as needed. The Care Coordinator I is responsible for care coordination in a primary care setting and serves in an expanded health care role to collaborate with the Integrated Care Team, which is inclusive of a Primary Care Provider, Nurse/Medical Assistant, Behavioral Health Provider, Community Health Worker, and patients/families to ensure the delivery of quality, cost-effective health care services and outcomes while closing gaps in the patient’s care. The Care Coordinator I will coordinate and evaluate all options and services with the goal of optimizing the patient’s health status and decreasing hospital and emergency room utilization.
Qualifications include:
− Bachelors Degree; or
− Associates Degree, and a minimum of 1 year of care management experience in behavioral health, social services, health care, and/or education; or
− High School diploma or GED and a minimum of 2 years’ care management experience in behavioral health, social services, health care, and/or education
Pre-employment background check, drug test and proof of COVID vaccination required and bi-lingual preferred. Salary commensurate with education, experience, licensure and/or certification(s). La Casa is an EOE.
