Licensed Practical Nurse- Supported Living and Supported Employment and Options

Horizons creates and sustains person-centered opportunities for people with special needs where they live, learn, work, and play. The Agency Nurse is to be responsible for ensuring the physical and emotional health and safety of each participant in Horizons Programs Inc. The Agency Nurse can facilitate requested trainings for agency staff as needed.

Essential Functions of the Job: *

  • Candidate must be able to lift at least 25 pounds
  • The successful candidate must be able to successfully utilize the training they will receive by Horizons in positive programming, augmented communication, and sensory awareness skills.
  • Physical ability to respond to critical incidents including but not limited to: evacuating participants from housing, public areas, vehicles, etc., if necessary.
  • Physical ability to negotiate the structural set up of the residences and agency campus, stairs, etc. as well as assist participants to do the same.
  • Ability to observe individual’s behavior, assess and direct appropriate medical treatment.
  • Ability to understand and follow all Agency policies and procedures
  • Physical ability to implement training and use equipment including but not limited to: all necessary medical equipment, items used for assistance in walking, etc. by participants, documentation required for all required licensing.

Responsibilities:

  • Participates in ongoing training required by the Department of Developmental Services, Horizons’ Policies and Procedures, employee professional goals, and to meet the changing needs of the program.
  • Follow the policies and procedures of Horizons including but not limited to being a mandated reporter of suspected abuse and neglect.
  • Communicate with participants in a manner they understand, such as total communication and sign language.
  • Timely arrival at assigned work location.
  • Converse with participants to reinforce positive behaviors and to promote socially appropriate interaction with peers and others.
  • Work under the direction and guidance of Agency Nurse to carry out assigned duties.
  • Conduct other health-related trainings for various parts of the agency or specific program trainings as directed by the Agency Nurse.
  • Have the best interests of each participant as the basis of your daily performance.

Supported Living / Individual and Family Support Services

  • Act as a link to community, emergency, medical, recreational and service providers.
  • Ensure that agency programs are in compliance with all DDS Licensing and Medication regulations under the supervision of the Agency Nurse.
  • Conduct quarterly audits of individuals’ medical records and report the findings for licensing review to the Agency Nurse.
  • Administer medication as directed by the Manager in accordance with Physician’s orders and DDS Medication Administration guidelines under the supervision of the Agency Nurse.

Requirements:

  • At least five years minimum experience working successfully in the medical field.
  • At least one year experience working with people who are developmentally disabled have autism, and exhibit challenging behavior is required or an approved equivalent.
  • Be at least 21 years of age, have an Associate Degree in Nursing or a Bachelor of Science degree in Nursing from an accredited school as well as a current, valid Connecticut Nursing License.
  • Must possess a valid Connecticut driver’s license and acceptable driving record.
  • Certification in CPR is preferred or must be successfully completed within the first 30 days of employment (Horizons offers these courses to employees).
  • Certification in First Aid is preferred or must be successfully completed within the first six months of employment (Horizons offers this course to employees – Waived for LPN and RN).
  • DDS Medication Administration Certification is preferred or must be successfully completed within the first year of employment.
  • File application, a letter of interest, at least 2 work references, 1 personal reference and current health form prior to working.
  • An acceptable background check and nursing license check performed by Horizons.
