This position cares for patients in the Adams, Allen, and Wells county areas.

POSITION OBJECTIVE:

Provides assessment and individualized occupational therapy programs to help Family Home Care patients develop, regain or maintain their ability to perform daily activities care consistent with Visiting Nurses Mission, Strategic Plan and Policies & Procedures.

QUALIFICATIONS:

  • Requires a Bachelor’s Degree in Occupational Therapy from a program accredited by the Council for Occupational Therapy Education.
  • The Occupational Therapist must be able to demonstrate skill in therapy practice, assess and respond to the needs of patients and families in varied settings, and cope with family emotional stress while respecting individual lifestyles.
  • Information management, Microsoft Office Products, and Internet abilities are required.
  • Must maintain a high degree of confidentiality and handle situations in a diplomatic and tactful manner.
  • Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of him/her, or to others in the workplace.

  • Must be comfortable working with hospice patients with the emphasis on palliative care.
  • Must have the ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, and/or governmental forms.
  • It is essential that you be able to accurately write reports, business correspondence, procedures, and meet documentation requirements using computer technology.
  • Strong interpersonal and assessment skills.
  • Must have Indiana motor vehicle operator’s license and have access to automobile with current automobile insurance coverage.
  • Must be able to drive independently to all patient/family homes.
  • Assist with inservicing the community on the hospice philosophy as needed/requested.
