Job Duties:
The Support Services for Veterans & Families (SSVF) Health Care Navigator will provide services that include: connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care, specialized case management, care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties. Incumbent will work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team. The Support Services for Veterans & Families Health Care Navigator as the representative of the American GI Forum NVOP will act as a liaison between the VA medical clinic and/or other community medical services. The Support Services for Veterans & Families Health Care Navigator will work with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans
Job Requirements:
The SSVF Health Care Navigator must have a Master in Social Work (MSW) with at least 2 years’ work experience in a health care or social services area of practice. Candidates with documented experience working with veterans or themselves a veteran or veteran spouse are preferred. Candidates must be willing to attend periodic training as required by the position. Candidates selected for the position must be available to begin immediately and carry professional liability insurance. Must be willing to travel within the local city in a privately owned vehicle. A valid driver’s license and vehicle general liability insurance is required. Must be able to pass a drug test, background check, and obtain a valid driver’s license. Clearance must be obtained prior to hiring.
