As a Team Coordinator you will be responsible for collaborating with our Patient Care Coordinators and IDG (Interdisciplinary Group) to ensure services provided by the team are in compliance with all federal, state, and local rules and regulations.

Your duties would include:

  • Assist the PCC in the coordination of clinical care and collaborates with members of IDG
  • Receives and processes patient referrals, as needed
  • Coordinates with the PCC all team activities, patient assignments, staffing patterns, and schedule to ensure patient and family needs are met in a timely manner
  • Ensures workflow tasks are completed promptly within electronic medical record (EMR)
  • Ensures all processes around patient care are followed to maintain compliance with federal, state, and accrediting body regulations
  • Ensures effective communication with all team members when there is a new admission of a change in the plan of care for a new or existing patient
  • Verifies release of information and requests medical records as appropriate
  • Receiving and scanning records into the EMR and notifying the PCC
  • Work in collaboration with the PCC to ensure that all Face-to-Face visits are scheduled timely, and the providers are notified promptly to ensure regulatory compliance.
  • Ensures that appropriate and thorough preparation is completed for the IDG meeting to ensure a compliant and timely meeting
  • Update an existing census and other administrative trackers for billing and leadership team
  • Collaborates with IDG members to ensure that all IDG notes are completed promptly, but no later than the end of each IDG meeting
  • Run reports in EMR, as scheduled, to ensure timely follow-up on missing items
  • Create and maintain IDG meeting agendas

Requirements include:

  • Hospice experience preferred
  • LPN License or CNA License (CNA must include admin/office experience)
  • At least (1) year of administrative/office experience
  • Computer Literate – this position will utilize many software programs
  • Homecare HomeBase experience preferred
  • Medicare/Medicaid rules and regualrion in relation to hospice preferred

Skills/Abilities/Knowledge:

  • Complies with acceptable professional standards and practices
  • EMR experience
  • Microsoft Office Suite experience (includes excel, word, and SharePoint)
  • Ability to work with standard office equipment (copier, fax, scanner, etc)
  • Excellent communication skills
  • Team oriented – we maintain a very collaborative environment in our office
  • Self-motivated, able to work with little supervision
  • Detail oriented
  • Organized

Pay: $40,000.00 – $45,000.00 per year
