We are looking for a collaborative and compassionate RN Case Manager to work with our interprofessional team in caring for patients with substance use disorders and perinatal behavioral health.
As part of our adult, outpatient, substance-use-disorder bridge program at the Grayken Center for Treatment, you will support physicians and clinicians. Duties include:
· Rooming patients and performing appropriate screenings, medication reconciliation and ordering
· Collecting drug testing and performing withdrawal assessments including COWs and CIWA
· Providing telephone support to patients, case management of patients and some routine follow-up visits
· Helping to coordinate referrals to community treatments resources
Qualifications:
· Graduate of an accredited School of Nursing; Bachelor’s degree preferred
· Licensed RN with Basic Life Support certification
· Five years of nursing experience; will consider candidates with less general nursing experience with addiction or bridge model experience
· Experience with Opioid Treatment Programs (OTP), Acute Treatment Services (ATS)/detox or clinical stabilization services (CSS) desirable
· Comfort working with people suffering from substance use disorders, responding to acute overdoses, providing naloxone and destigmatizing medications preferred
