Summary

POSITION SUMMARY:

The Telehealth RN is responsible for the clinical review and interpretation of patient monitored data and acts on significant changes in the patient’s condition.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Recognizes individual patient safety concerns and conveys that information to the appropriate patient care staff.
  • Reacts appropriately to potential emergencies as indicated by abnormal monitoring results.
  • Obtains verbal orders from physicians as appropriate when establishing a change in the care plan .
  • Prioritizes actions appropriate to the patient’s needs and level of acuity.
  • Helps to set parameters that are meaningful for the patient with the physician’s approval.
  • Documents patient incidents according to policy.
  • Complies with safety policies and procedures at all times and informs the Telehealth Manager of any unsafe conditions.
  • Recognizes emergencies and demonstrates knowledge of emergency procedures.
  • Is familiar with all models of monitoring equipment and related peripherals.
  • Can instruct in their operations verbally in order to troubleshoot malfunctions in the field.
  • Teaches staff, students, patients, and families the value of daily monitoring, including improved patient outcomes.
  • Instructs on the installation and use of monitoring equipment.
  • Cooperates in the orientation of new staff persons by acquainting them to the Telehealth Clinician RN position as requested.
  • Participates in staff meetings and in-service training.
  • Reviews all incoming monitoring data.
  • Effectively communicates changes in the patient’s condition to the appropriate patient care staff so that intervention can occur in a timely manner.
  • Notifies the patient’s physician of significant changes in the patient’s condition when the Case Manager is not involved.
  • Communicates with internal staff, physicians, and insurers in order to prompt timely patient care, to change the patient’s plan of care as needed, and generally to collaborate in order to keep the patient in the least restrictive environment possible.
  • Faxes tabular trends to physicians monthly and when the patient’s condition warrants.
  • Identifies patients that may be appropriate for monitoring or that may benefit from other monitoring programs after discharge from home health care.
  • Follows the organization’s policy regarding the confidential treatment of patient, employee, and proprietary information.
  • Identifies areas which may present compliance problems with federal regulations for the position’s assigned responsibilities.
  • Follows the organization’s policies and procedures in order to maintain compliance with federal regulations.
  • Suggests monitoring procedures and reports suspected violations.
  • Keeps internal and external clinicians informed of changes in the conditions of monitored patients and suggests interventions as appropriate.
  • Helps other clinicians to incorporate daily monitoring into the patient’s plan of care.
  • Is an expert at using the monitoring central station software.
  • Effectively uses software to provide information, document changes and actions, and keep all patient care records up-to-date.
  • Signs of on verbal orders taken by the Telehealth LPN .
  • Uses strong clinical knowledge and verbal assessment skills to assess patient’s condition remotely .
  • Other duties as assigned.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

  • Currently licensed as a Registered Nurse in Pennsylvania
  • Valid Pennsylvania driver’s license
  • One year previous work experience as RN in med/surg, hospital, or related skilled setting.
  • Strong clinical knowledge of cardiopulmonary disease processes .
  • Strong clinical knowledge of cardiopulmonary pharmacology .
  • A health care provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e. not excluded from participation in Medicare, Medicaid or any other federal or state health insurance program.

PREFERRED QUALIFICATIONS:

  • Two year’s work experience as RN in a critical care setting or a cardiac telemetry inpatient setting.
  • Two years home care experience
