Job Details
**Licensed Vocational Nurse in the State of California.
**One year experience as an LVN.
**Knowledge of the principles and practices of primary medical and nursing care. Understands the philosophy of hospice concept preferred.
**Will Coordinate all services not provided in the SNF
**Coordinates with healthcare providers for admissions to SNF
**Completes Discharge on admissions
**Solid organizational skills.
BLS required within 60 days of hire.
Current California driver’s license and proof of insurance.
