POSITION OVERVIEW:
As a OneCare Kansas (OCK) Community Medical Care Manager, you will connect qualifying Kancare Medicaid members with necessary services to help them improve their health and stay as healthy as possible. OCK services begin with a team who will learn about the needs that affect both the physical and mental health needs of the members enrolled in OCK. This service can be provided in the member’s home or in another provider’s office. You would be responsible for assembling a team of providers to work with the consumers to set goals and make plans to meet them.

A Care Management Team includes the members and their medical providers, caregivers, Care Managers (nurses and social workers), pharmacists and HME providers, mental health specialists, and other healthcare specialists.

As a OCK Community Medical Care Manager, the team you assemble helps the members set health goals to improve their health and quality of life. The team works closely with the members and their medical providers to:

  • Work with the members to help them understand and participate in their plan of care.
  • Set goals with them that will help them improve their health.
  • Review the medications to be sure they know how to use them correctly.
  • Ensure that members have access to the correct medications and home medical equipment.
  • Facilitate follow up care with providers after a hospital admission or emergency room visit.
  • Help members resolve any barriers to attending medical appointments, especially after a hospital or emergency room visit.
  • Provide the members and their caregivers with tools, resources, and support to them better manage their health.

The OneCare Kansas Program at MHA (Mental Health Association) is a whole person model of person-centered care placing the member at the center of the care team. The focus is always on the consumer and is assembled with the goal in mind to:

  • Improve the health and quality of life for those in our community living with chronic medical and mental health conditions.
  • Empower members to take care of their long-term health concerns and conditions.
  • We work closely with members’ primary care doctors and other specialists to ensure continuity of care and communication among all providers.
  • Find community resources needed by the members and their families.
  • Connect members to peers, in both group and individual settings, who have a shared personal experience and can support the member in meeting their health goals.


POSITION REQUIREMENTS
:

  • Expected to have a BA/BS degree or four years of equivalent education and/or experience working in the human services field.
  • Intermediate to Advanced computer literacy required.
  • Preferred areas include knowledge of community resources, housing alternatives and vocational services; experience working in the community with individuals who have chronic illness such as serious and persistent mental illness, diabetes and asthma; ability to write and communicate verbally in a clear and concise fashion; and the ability to develop and maintain rapport with consumers, constituents and staff.
  • Valid Kansas driver’s license and access to personal vehicle required.
  • Intermediate to advanced proficiency in Microsoft Office including Word, Excel, and Outlook.
  • Experience with electronic health records is a plus.
  • Ideal candidate will possess excellent time management and organizational skills.


PHYSICAL REQUIREMENTS

  • Driving for purpose of community mobility
  • Bending/stooping
