JOB DESCRIPTION

SUMMARY: Full time Position (32-40 hours per week)

Under general supervision, provides either inpatient or outpatient patient care services for nursing home and Seniors. Provides mobile phlebotomy, chart review, assistance with telehealth appointment scheduling and remote patient monitoring enrollment.

DUTIES AND RESPONSIBILITIES:

The Contractor will provide the following services:

 Provide telephonic outreach, inhouse nursing home screening and home visits to targeted patients to facilitate enrollment into the Healing Our Village Transitional Care program.

Provide blood draw, collect urine and on-site screening for height, weight, BMI, blood pressure, blood glucose and blood glucose.

 Provide training in the appropriate use of remote patient monitoring equipment (blood glucose monitors, blood pressure cuffs, weight scales,etc.)

 Provide BASIC diabetes education using a standardized script.
 Facilitate telehealth appointment with the physician groups and Clinical Pharmacist
 All other duties as assigned.

MINIMUM JOB REQUIREMENTS:

Certificate from accredited Medical Assistant or Phlebotomist
Certified Diabetes Prevention Lifestyle Coach (Preferred )
Valid Driver License with insurance
Bilingual preferred

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

 Skill in the use of personal computers and related software applications.
 Ability to effectively communicate with patients and their family members
 Friendly and outgoing personality

WORKING CONDITIONS AND PHYSICAL EFFORT:

 Work is normally performed in the field (home visits) and at the pharmacy (making phone calls).

 No or very limited physical effort required.
 No or very limited exposure to physical risk.

Job Types: Contract, Full-time

Pay: $33,453.00 – $34,785.00 per year
