Infusion Nurse Navigator – US Digestive Health – Lancaster, PA

JOB DESCRIPTION: NURSE NAVIGATOR

REPORTS TO: Director of Nursing & Operations

RESPONSIBLE AND ACCOUNTABLE FOR:

  • Acting as a liaison between the providers and the patient
  • Works closely with physicians in various specialty areas
  • Coordinates patient’s care plan communication and program
  • Provide patient with education related to their illness and planned treatment
  • Triage patient calls to appropriate processes
  • Assumes the role of patient advocate
  • Making every effort to remain a flexible, cooperative and supportive member of the USDH office staff

PROFESSIONAL REQUIREMENTS:

  • Registered Nurse and/or Licensed Practical Nurse in the state of Pennsylvania with a minimum of 5 years nursing experience
  • Should have at least 2 to 3 years of experience in nursing , as a health or patient educator, or a pharmaceutical representative
  • For special cases should have a Master’s degree in nursing or public health with minimum of 2 years work experience
  • Possesses the ability to deal tactfully and effectively with patient and their families, other employees, and physicians
  • Strong people skills
  • High level of telephone skills with good communication ability

TYPICAL PHYSICAL AND MENTAL DEMANDS:

  • Full range of body motion. Manual and finger dexterity and eye-hand coordination
  • Frequent mobility and/or sitting required for extended periods of time. Some bending, lifting, and stooping required
  • Normal color perception and corrected visual acuity
  • Involvement with management of information concerning physician, staff, hospital personnel, and patients. Occasional high stress work may require dealing with angry, demanding patients and/or personnel

TYPICAL WORKING CONDITIONS:

  • Clean environment in a gastroenterology physicians’ office

  • Keeps all patient information, practice information, and staff information strictly confidential
  • Performs routine work under general administrative direction

JOB DESCRIPTION

  • Follow established procedure, referring all new or unusual questions/concerns to a manager
  • Acts as a patient advocate, ensuring a setting that protects the rights of the patient and provides an atmosphere that does not compromise the patient’s physical or mental well-being, safety, or dignity
  • Demonstrates knowledge of OSHA, Code Blue, and Fire Safety measures.
  • Maintains knowledge of, and complies with, established policies and procedures.
  • Attends all required meetings

SPECIFIC:

  • Requires advanced practice expertise to identify and implement improvement processes, and the ability to design, direct, and implement health care plans
  • Works compassionately, professionally, and courteously with patients, their families, physicians, and fellow USDH staff
  • Provide emotional support and counseling for patients related to the clinical situation
  • Works with a multidisciplinary team to develop and implement an up-to-date care plan
  • Communicates with all members of the healthcare team on behalf of the patient
  • Demonstrates ability to tactfully handle difficult situations, while maintaining a positive attitude
  • Use clinical protocols to refer patients to the appropriate specialist for diagnosis or treatment
  • Help patients and their families in obtaining referrals to a specific specialist
  • Work with designated physicians to develop and maintain hospital protocols
  • Directly involved with the development and enhancement processes with the aim to improve the clinical experience for referred patients or physicians
  • Communicates with referring physician’s offices as required by the patient and physician
  • Work with marketing and outreach departments to educate referring physician facilities on services
  • Demonstrates good judgment and reasoning when investigating and solving problems
  • Maintains detailed, thorough and accurate documentation of all phone conversations in patient’s record and have signed off by both physician and nurse practitioner, as applicable
  • The provider (physician/nurse practitioner/physician assistant) is then notified of medical problem (s) either in writing or by phone depending on urgency of problem (s). A response is then called to the patient with the provider’s instructions
  • Identifies an emergency situation and is able to effectively triage problems
  • Call, e-mail, or fax prescription refills or renewals to pharmacies as per provider orders
  • Directs the chart to the scheduling department for scheduling of outpatient procedures
  • Under supervision from providers, give results on pathologies, x-rays, and lab studies to patients either verbally, by mail, or Lab Call system
  • Consistently shows ability to recognize and deal with priorities
  • Should have good analytical and deduction skills

ATTENDANCE AND RELIABILITY:

  • Does not abuse or take advantage of sick time. Regular attendance is a necessity.
  • Reports to work as scheduled and is able to complete all functions of the job.
  • Always provides proper notification and advance notice of absences.
  • Reports to work, is at the assigned station, and ready to work at their starting time.
  • Returns from 30 minute lunch on time

OTHER:

  • Maintains a well-groomed and appropriate appearance
  • Maintains a clean, well-kept work area
  • Performs other duties as required by the Direct of Nursing and Operations

NOTE:

  • All staff will be sufficiently in serviced to each procedure prior to performing that task
