Better Living Home Health Care is a locally owned and operated Medicare and Medicaid certified home health agency with the mission of keeping our clients happy and healthy at home. We provide high quality care to seniors and people with disabilities. Quality care is provided through a team of registered nurses, companions, aides and physical and occupational therapists. We are looking to hire passionate and caring staff members for our clients to work one-on-one with them in their homes. We serve the counties of Daviess, Dubois, Gibson, Greene, Knox, Martin, Orange, Perry, Pike, Posey, Spencer, Warrick and Vanderburgh.
- Performs all physical therapy procedures, planned and delegated, as prescribed by the physician in the client’s home when instructed and supervised by a qualified Physical Therapist
- Instructs, motivates and assists clients in learning and improving functional activities
- Observes client’s progress and response to treatment and reports findings to the physical therapist
- Prepares clinical notes and progress reports documenting physical therapy treatments performed and submits them for review to the supervising Physical Therapist for inclusion into the clinical record
- Reports any changes in the client’s condition to the qualified supervising Physical Therapist and Agency Management
- Changes in the plan of care may be made following authorization by the client’s physician and subsequent authorization, instruction and supervision by the Physical Therapist
- Participates in case conferences, staff meetings, and in-service programs as required
- Submits all documentation (clinical and payroll) timely
- Assists with quarterly record reviews if requested
- Follows directives of Case Manager and supports the efforts of case coordination
- All other duties assigned
- One year PTA experience, with 6 months of that being home health experience providing PTA services to clients or as required by state law
