Requirements:
- Must be licensed as an Occupational Therapist in Arizona
- Must possess a Valid Drivers License
- Must possess or be able to obtain Level 1 Arizona Fingerprint Clearance Card
- Must have reliable transportation
- Must be willing to travel to members homes or community locations for ongoing treatment
- Must be open to teletherapy options based on members preference
Duties:
- Evaluate individuals for occupational therapy services
- Develop a Plan of Care based on the members needs and goals allowing them to reach their full potential
- Review and Update Plan of Care Annually
- Provide ongoing treatment for individuals as outlined in Plan of Care
- Document SOAP notes electronically
- Complete Client Progress Reports electronically
- Supervise COTA’s
- Effectively communicate with clients, families, administration and COTA’s, etc.
- Excellent written and verbal communication skills
- Must be a Self-Starter
- Spanish speaker is a strong plus

Competitive Hourly Range of $59.00 – $96.00.
