Summit Healthcare Services is a well-established agency providing innovative healthcare through our one-of-a-kind progressive continuum of care. Summit offers services for clients in any stage of aging, from Living Well, Home Care and Therapy to Skilled Home Health, Palliative and Hospice. We are currently seeking an experienced Full-time Director of Clinical Services for our Hospice operations located in Phoenix, AZ. Working collaboratively with our Vice President of Clinical Services, you will oversee and supervise an interdisciplinary team in the delivery of all Hospice care to patients and families. JOB REQUIREMENTS

  • 5 years of Hospice nursing experience
  • At least 2 years in a leadership role / managerial position
  • Excellent understanding of Medicare Hospice regulations

PRIMARY RESPONSIBILITIES

  • Manage the day-to-day operations of a robust Hospice program leading the effort to provide quality patient care.
  • Supervise the planning, coordination and delivery of the interdisciplinary group interventions.
  • Ensure proper follow-through of all Hospice referrals and admissions.
  • Participate in and provide leadership to the interdisciplinary team in all IDG meetings, in the development, review and revision of the patient/family plan of care, facilitate problem-solving sessions and the delivery of team support.
  • Ensure compliance with federal, state, and local rules, accreditation standards and Hospice policies in all patient care aspects of the Hospice.
  • Serve as a member of the management team in order to address problems associated with the delivery of services, staff development needs and to put forth suggestions for appropriate additions/changes in policies and procedures.
  • Recruit, hire, train, and mentor staff.
  • Conduct appropriate supervisory visits, which allow for the clinical observation of the performance of assigned team members.
  • Supervise the maintenance of medical records assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.
  • Oversees Hospice’s ongoing Quality Assessment/Performance Improvement (QAPI) program.
  • Develop a cooperative relationships with and communicate effectively and professionally with physicians.

Pay: $95,000.00 – $115,000.00 per year
