LPN (Licensed Practical Nurse) – Everest Hospice – Goodyear, AZ

Job Summary:

The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their home under the general guidance and direction of a Registered Nurse and in accordance with Agency policies and procedures, and following established standards and practice.

Pay is based on experience.

Qualifications and Experience:

Ø Currently licensed in the State of Utah as an LPN.

Ø One year of nursing experience and experience in home health care recommended.

Ø Valid State driver’s license and reliable automobile.

Ø Proof of current automobile insurance.

Ø Ability to read and communicate effectively in English.

Ø Additional languages preferred but not required.

Accountability: Reports directly to the Director of Nursing.

Typical Physical Demands:

Requires full range of body motion including handling and lifting of patients. Manual and finger dexterity, and hand-eye coordination. Occasionally lifts and carries items weighing up to 30 pounds or greater. Requires corrected vision and hearing to normal range. Requires working under stressful conditions or working irregular hours. Requires exposure to communicable diseases. Requires a current drivers’ license and the ability to drive and access a patient’s home without an assistive device.

Typical Working Conditions:

Majority of work is performed in providing patient care services in the patients’ home. Exposure to communicable diseases is common.

Job Duties:

Ø Provides skilled nursing in the home as ordered on the plan of treatment and nursing care plan. Helps develop plans of care.

Ø Evaluates the significance assessment findings and communicates any pertinent information about the patient’s clinical status and ongoing needs to others in a timely fashion.

Ø Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.

Ø Demonstrates knowledge of medications and their correct administration based on age of patient and patient’s clinical position.

Ø Only administers prescribed medications and treatments according to law and as permitted within the scope of their license.

Ø Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.

Ø Functions as a team member demonstrating collaboration with, and responsibility to patient, physician, and the interdisciplinary team.

Ø Adheres to all agency, payroll, billing, and documentation policies and procedures.

Ø Writes clinical notes for each visit in the individual patient record.

Ø Participates in agency’s other mandated activities, as requested by Clinical Supervisor or Director of Nursing including in-services and UR/PI committee.

Ø Performs patient care responsibilities considering needs to the standard of care for patient’s age and teaches self-care techniques to the patient, family, or both.

Ø Under rare and extenuating circumstances, LPNs may be requested to take on the duties of a CNA. If this circumstance arises, the LPN will continue to be paid their LPN rate as agreed upon in their payroll agreement.

Ø Addresses and supports cultural practices, as long as such practices to not harm others or interfere with the planned course of medical therapy.

Ø Promotes patient/family/caregiver education using various verbal and written communication techniques that take into account the patient’s/family’s cultural, ethnic, and/or personal needs or preferences.

Ø Maintains regulatory requirements, such as: valid license from the Utah Department of Commerce.

Ø Maintains patient confidentiality at all times.

Ø Reports to work on time and as schedules, completes work within designated time.

Ø Completes in-services and returns in a timely fashion.

Ø Attends in-services, as schedules.

Ø Attends required staff meetings annually, reads and returns all monthly staff meeting minutes.

Ø Represents the organization in a positive and professional manner.

Ø Complies with all organizational policies regarding ethical business practices.

Ø Communicates the mission, ethics, and goals of the organization.

Ø Adheres to dress code, maintains neat and professional appearance, wears identification (as required.)
