Position Summary:

As an LPN, you’ll use your expertise to provide professional nursing care to geriatric patients with the overall goal of helping them to regain independence. We collaborate to provide an interdisciplinary approach to caring for our patients in home or senior living facilities in accordance with the Plan of Care, agency guidelines and the State Nurse Practice Act.

Required Experience:

  • You are a graduate of an accredited school of professional nursing with a current license for the State of Michigan and have at least one (1) years of field experience, preferably in a home health care setting.
  • You are willing to travel.
  • You are passionate about working with elderly and helping them achieve great outcomes.
  • You love to be part of an interdisciplinary team, laugh, and have fun while working hard to provide excellent service.
  • Proficient in general computer skills.

Responsibilities:

  • Provides skilled nursing visits for clients as directed by the Case Manager and follows the care plan established by the Registered Nurse.
  • Recognizes and reports changes in client condition to the Case Manager, Physician, and Clinical Supervisor as directed.
  • Communicates information on a timely basis, consistent with Agency policy and urgency of the situation. Communicates verbally, as needed, and through complete and accurate documentation in the clinical record.
  • Participates in coordination of home care services, attends meetings as required, and follows reporting guidelines.
  • Performs other duties as assigned.

General Working Conditions:

This position entails standing for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires the ability to review detailed documents and read computer screens. The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. Occasional travel to different locations may be required.
