Under indirect supervision from Clincial Care Manager Supervisor, a Care Manager will work with clients with identified Mental Health, Substance Use Disorders, or Intellectual/Developmental Disabilities, to assist with providing care management assessments to determine service needs, strengths, resources, preferences, and goals to develop a Care Management Plan, provide referral and linkage to medical, behavioral health, and social support programs and services to achieve goals, and reduce fragmentation of care and maximize outcomes.
Candidates will need to be comfortable working both in the office, and in the community to meet client needs.
Education and/or Experience:
A Bachelor’s degree in a human service field with two years MH/SA/DD experience with the population served; or, a licensed RN with two years MH/SA/DD experience with the population served.
This position is also open to a candidate with a Master’s degree in a human service field with two years MH/SA/DD experience with the population served; who may also have licensure as an LCSW(A), LCAS(A), LMFT(A), LCMHC(A).
Candidates who are Bilingual (English/Spanish) are highly encouraged to apply.
Trainings will be required to completed as assigned.

Pay: $40,000.00 – $50,000.00 per year
