Wound Care Consultants is recruiting for nurse practitioners to treat residents living in long-term care facilities and at home. Candidates must have experience providing wound care as a nurse practitioner in the long-term care environment. Moreover, the ideal candidate will be prepared to provide medical services as an independent contractor, can expect to be highly compensated, and should be prepared for at least a moderate level of travel. The provider will generally round 4-5 days a week and on a schedule that accommodates both the provider and the facility. The work is located in long-term care facilities in the Beachwood, Ohio area.
Job Summary:
1. Working collaboratively with the health care team in order to deliver wound care services to residents of long-term care facilities.
2. The wound care nurse practitioner will provide thorough assessments, diagnosis, clinical decision making, treatment and prescribing, and direct management of acute and chronic wounds.
3. A predominant focus on healing wounds swiftly, infection reduction (antibiotic misuse) and minimizing re‑hospitalization.
4. Employing a comprehensive wound care solution that mandates strict adherence to risk assessment protocols and the provision of individualized interventions that focus on moisture management, pressure redistribution, turning and repositioning and mobility and nutrition.
5. Evaluating and treating chronic and acute episodic wounds, minor injuries, accidents, and other injuries/illnesses, from debridement of wounds to application and management of topical wound management products.
6. Providing timely and targeted clinical intervention, urgent treatment, health care provider education and consequential disease prevention, facility support and validation in meeting state and federal health care requirements.
7. Coordinates the patient plan of care with the physician and the facility’s clinical disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient’s medical condition to ensure healing outcome goals.
8. Provide in-home wound care services to individuals at multiple private residences, assisted living and independent living facilities.
9. Recommendations for prescription of required medications and/or Durable Medical Equipment to facilitate wound prevention and healing.
10. Educate, support and collaborate with facility staff to reduce off-site treatments, in-house costs and re-hospitalization.
