Trillium Health Resources is a leading specialty care manager for individuals with substance use, mental illness and intellectual/developmental disabilities in eastern North Carolina. We have a career opening for a Transition Integrated Care Nurse.

This position is responsible for providing Care Coordination/Management (treatment planning, assessment, monitoring, educating and referral/linking) to individuals identified in the Transition to Community Living (TCL) Program Special Needs Population by targeting those with chronic, unresolved or complex physical, behavioral health and social determinant needs. In the Transition Integrated Care Nurse role, you’ll work on an interdisciplinary team to address the functions of the Olmstead Settlement/DOJ Settlement Agreement while meeting the needs of the community.

On a typical day, you might:

  • Work collaboratively with other licensed Clinicians, Health Care Professionals and Peer Supports to ensure successful discharge/transition planning/post transition monitoring.
  • Assess member’s needs who have transitioned into supportive housing to determine if additional supports and services are needed for continued independent living.
  • Ensure discharge/transition planning and diversion occur in a timely and effective manner.
  • Review clinical documentation such as comprehensive clinical assessments and psychological evaluations to ensure appropriate recommendations for services and level of care.

Qualifications:

Education: Fully licensed by the North Carolina State Board of Nursing as a RN.

Experience: Minimum of two (2) years documented experience working with the co-occurring of physical health and behavioral health, physical health or chronic condition population, preferably in all 3 areas.

License/Certification: Fully licensed by the North Carolina State Board of Nursing as a RN.

Must have a valid driver’s license

Must reside in NC
