We serve patients throughout eight counties in Central Alabama providing numerous services that are non-traditional hospice services. Our programs include a pet therapy dog, pet peace of mind program, comfort touch massage therapist, music therapist, music and memory program with a patients favorite songs loaded onto an MP3 player for their personal listening, barber service, food boxes and numerous other services for patients on a limited income.

JOB DESCRIPTION:

-RN Hospice Case Manager – We are hiring!

Non-exempt (Full-Time)

Responsible to: Director of Patient Care Services & Medical Director

General Summary: Under physician’s orders and upon admission to Lakeside Hospice, the Hospice nurse is responsible for identifying patient / family Hospice care needs for providing nursing and supportive care to terminally ill patients and families in their homes. Abides by the philosophy, policies, and procedures of Lakeside Hospice, Inc.

Responsibilities and Functions: On admission completes initial nursing assessment and pain assessment including physical, emotional, and mental status. This is to be completed within 24 hours of admission.

  • Initiate communication with attending physicians, other hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for the patient / family.
  • Prepares, implements, and coordinates treatment plan for patient / family with team members. Evaluates and updates treatment plan every 14 days or as needs of the patient / family change.
  • Teaches family members patient care skills and encourages their participation including: pain control, medication (the RN titrates within range prescribed), injections, dressings, hygiene, nutrition, checking vital signs in accordance with orders from physician, and identifies any emotional needs.
  • Assists and supports patient / family through physical and / or emotional crisis situations.
  • Maintains up-to-date patient records so that problems, interventions, and goals are accurate and clearly stated and changes are reflected as they occur. Reviews charts of patients for whom they are case managers regularly for completeness. Peer review as designated.
  • Performs final chart review upon death or discharge of patients for whom they are the case manager.
  • Meets and participates regularly with Interdisciplinary Team meeting.
  • Sensitive to the needs of patients and families while maintaining appropriate professional boundaries.
  • Ability to cope with high stress of continual multiple losses (deaths) and family dynamics.

Physical Demands / Traits:

  • Must be able to express or exchange ideas by means of the spoken word.
  • Must be able to perceive the nature of sounds by ear.

  • Frequent lifting and positioning of patients.
  • Frequent local travel.

Qualifications:

  • Possession of current license to practice as a registered nurse in the State of Alabama.
  • RN Degree
  • Demonstrated ability to assess and respond to the needs of patients / families in varied settings.
  • Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual beliefs.
  • Knowledge of Hospice Medicare Guidelines and Hospice concept of care.
  • Current Alabama Driver’s License.
  • Proof of current auto insurance.

Pay: $29.00 – $33.00 per hour
