About This Palliative Care Consultant (NP) Position

HopeHealth is seeking an experienced nurse practitioner to join an existing team providing home-based care as an extension of primary care practices.

This person would deliver care to patients living in the community who are unable to access care by going to the office. Most of the population served is seriously ill with complex medical conditions, and a subset have significant psychiatric issues or other physical limitations that make travel to an office complicated.

The Palliative Care Consultant (NP) will work in collaboration Primary Care Physician (PCP) and their office staff like case managers to recommend a plan of care focused on the relief of pain and symptoms, determination of disease trajectory, establishing goals of care.

One of the primary goals of this program is to enhance patient and PCP experience of care and reduce unwanted and unnecessary medical care like hospitalizations and ED visits.

This person would join an existing full time NP and a part-time social worker. This position is 20 hours/week with some flexibility to work 2-3 days/week on an alternating schedule.


What will you do?

Duties / Responsibilities

  • Obtains medical/health histories and performs physical examinations and provides follow-up care for patient.
  • Diagnoses, treats and monitors acute and chronic conditions in collaboration with PCP.
  • Assesses severity of illness and actively elicits goals of care
  • Expertise in managing burdensome symptoms like pain, nausea, fatigue, breathlessness
  • Identifies hospice eligible patients and offers hospice as an option if appropriate
  • Takes a team approach to care


What are the qualifications for this job?

  • Graduate of an accredited nursing school
  • Licensed as a Registered Nurse in Massachusetts required
  • Minimum of 1-2 years of clinical experience
  • Excellent assessment, clinical and communication skills
  • Experience with wound care, medication management and cardiac care a plus
