Job Summary: Primary functions are to administer skilled nursing care for clients of all ages in their place of residence after normal business hours, coordinate care with the interdisciplinary team, patient/family and referring agency.
Essential Functions:
* comprehensive assessments of the terminal disease on the patients physical, functional, and environmental needs and ADL’s, risk for grief, cultural and spiritual, verbal and non-verbal.
*follow the individualized POC and recommend revisions to the plan as necessary
*Consulting with and educating the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition and dietary needs.
*provide appropriate preventive nursing procedures as directed by the Hospice Physician.
*Informing physician, and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care.
*Timely documentation with in the assigned EMR.
Job Type: Full-time
Pay: $80,000.00 – $85,000.00 per year
