About:

The nonprofit San Francisco Campus for Jewish Living serves a significant number of patients and long-term care/skilled nursing residents each year in its Jewish Home & Rehab Center. Non-residential care is offered through our short-term and rehabilitation services unit for patients who require specialized interdisciplinary rehabilitation to recover from surgery or an acute illness, and through our acute geriatric psychiatry hospital. With a complement of close to 650 compassionate, experienced clinical and allied health personnel, SFCJL provides older adults with a variety of life-enriching healthcare programs and services in a welcoming, stimulating environment.

SFCJL’s transformed campus includes the new Lynne & Roy M. Frank Residences (assisted living apartments and memory care & support assisted living suites), and Byer Square, a non-residential community of programs, services, and support.

Position Overview:

The Recreation Coordinator – Acute Geri Psych (AGPH Recreation Coordinator) maintains patient activity programs, alternative treatments, communicates with patients, families, surrogates, conservators, physicians, and other health team members. The AGPH Recreation Coordinator works in conjunction with all members of the Interdisciplinary Team to provide active treatment to help accomplish patient goals. The AGPH Recreation Coordinator assists with the completion of mandatory documentation as needed. The AGPH Recreation Coordinator works under the supervision of the Director of Recreation Therapy.

Essential Functions:

  • Assessment / Reassessment: Assures that an initial activity assessment and admission note is completed within 72 hours of admission of patients under the supervision of the Director of Recreation Therapy or CTRS designee.
  • Assures that a progress note is completed with any change in behaviors, change in condition, and at least weekly.
  • Treatment Planning: Assures that the treatment plan is multidisciplinary and includes individualized therapeutic interventions within 72 hours of admission under the direction of the Director of Recreation Therapy or CTRS designee.
  • Interdisciplinary Team (IDT): Actively participates in the IDT process, as needed, under the direction of the Director of Recreation Therapy and is prepared for the presentation of pertinent information during IDT including revision of treatment plans, behavioral interventions, etc.
  • Communication: Provides information about patients to the Director of Recreation Therapy verbally and in writing utilizing the Recreation Therapy Treatment Note.
  • Documentation: Maintains accurate documentation under the supervision of the Director of Recreation Therapy or CTRS designee including but not limited to:
  • Antecedents to inappropriate behaviors, changes in cognition / physical status, etc., and reports to licensed nurse and/or physician for further review and possible change in treatment, etc.
  • Admission assessments & admission note
  • Progress note at least weekly
  • Individual and group participation Treatment Notes
  • Treatment plan.

Therapeutic Treatment Groups: Provides group and one on one visit on a pre-scheduled and as-needed basis including but not limited to:

Works with the recreation therapist to maintain a daily / monthly activity calendar.

Leads and/or co-leads group activities including but not limited to: expressive arts therapies, coping skills, recreational, social, spiritual, educational, leisure skills, etc.

Accounts for all sharps, equipment, and supplies at the end of each group.

Secures all materials in a locked area at the end of each group session.

Patient Teaching: In conjunction with the Director of Recreation Therapy, instructs patients/patients ’ authorized representative/conservator regarding treatment plans including leisure activities.

Notification:

Notifies the Director of Recreation Therapy, charge nurse, and/or IDT of new behavior problems promptly.

Reports incidents of elder / dependent abuse per facility policy.

Removes any defective equipment from immediate use using the “lockout – tag out” procedure and notifies the appropriate department/vendor for repair.

Notifies Housekeeping of any spills etc. promptly.

Completes a facility incident report when unusual occurrences happen, participates in the investigation, and notification to the appropriate facility leader.

Perform other duties as assigned.


Qualifications:

  • Minimum BA/ BS degree preferably in recreation, social service, or related field
  • Two years experience working in the field of geriatrics preferred.
  • A minimum of one year of experience working in an acute psychiatric hospital is preferred.
  • CTRS or Activity certification is preferred
  • CPI – Nonviolent Crisis Intervention (completed within 3-6 months of start date.)
  • Ability to perform individual and group therapeutic activities and collaborate with others.
  • Ability to interact effectively with patients, visitors, administration, physicians, and staff.
  • Knowledge of State and Federal laws and regulations governing Mental Health.
  • Ability to read, write and speak English.
  • Desire to work with geriatric and mentally disabled persons.
  • Ability to generate enthusiasm and cooperation from others.
  • Must possess the ability to make independent decisions when necessary.
  • Ability to understand and respond appropriately to cultural diversity.
  • Ability, under the direction of the Director of Recreation Therapy or CTRS designee, to complete all mandated documentation promptly.
  • Comfort in creating and updating forms/assessment utilizing a computerized charting system
