THE OPPORTUNITY
Providing virtual annual physical assessments for geriatric patients throughout the state of New York. These are one-time, 90 minute assessments with no diagnosing or prescribing.
- 3 year contract;
- 1099 contract position;
- Malpractice insurance covered by company;
- 2-3+ days a week (Saturday and Sunday availability, if desired);
- BYOD, bring your own device.
WHAT YOU’LL DO
See geriatric patients using the technology platform provided by the practice; Provide excellent, personalized patient care according to the guidelines provided by the practice; Identify gaps in care; Document pertinent and active medical conditions; Support the physician/patient relationship and ensure timely and adequate communication, documentation of assessment findings, recommendations, need for additional services, emergency services required if necessary and need for follow up and timelines for follow up to primary care provider (PCP) and health plan as required; Determine emergent situations requiring escalation and take appropriate action; Knowledge of Evidence-based Clinical Practice Standards; Make general recommendations to members intended to improve members’ knowledge of their chronic condition(s), such as information concerning recommended testing; Adhere to all confidentiality and HIPAA. All exams are 90 minutes in length.
WHAT YOU’LL NEED
- New York Medical License;
- 2 years (3600 hours) of experience;
- ANCC or AANP Board Certification;
- Strong organizational skills and an attention to detail;
- Excellent communication;
