Job Title: Home Health Admission & OASIS Registered Nurse (RN)

Compensation: $150 per visit + mileage reimbursement

Home Health Admission & OASIS RN Responsibilities:
- Perform Admission / “Start of Care” OASIS Visits throughout an area of the Metro
- Evaluate and assess clients for home health services and create their plan of care
- Recommend services from other disciplines that have not yet been ordered by the physician
- Collaborate with clients, families, physicians, and other members of the home care team
Home Health Admission & OASIS RN Qualifications:
- Licensed as a Registered Nurse in the state of Minnesota
- Driving is required; must have a reliable vehicle with current insurance and a valid Driver’s License
- Prior Home Health Nursing experience
- Proficiency with Medicare & OASIS documentation
- Demonstrated ability to work autonomously
- Excellent at planning, organizing, and communicating
