PHYSICAL THERAPIST
Farmingdale Physical Therapy West is a leading provider of outpatient physical therapy and rehabilitation services with two locations on Long Island, conveniently located in Bethpage and Ronkonkoma. We provide both in clinic and home care based services.
We are a dynamic, evidence-based physical therapy clinic specializing in orthopedic and sports medicine. We are growing and need a Physical Therapist to join our team. The right candidate will be able and willing to provide Home Care evaluations and services to our patients in both Nassau and Suffolk Counties.
The right candidate needs to be a self-motivated, very organized individual with an advanced degree in physical therapy from an APTA-accredited physical therapy program, preferably with 1-2 years’ experience. Individual must have a current New York state license in good standing with the New York State Board of Physical Therapy.
Duties and responsibilities include but are not limited to:
Evaluating each patient (utilizing our 7-step exam) that is specific to age, illness, injury, and/or disease.
TIMELY preparing a written plan of care (using our web based EMR) based on evaluation findings.
Documenting PQRS and functional outcome tools as required by our payors.
Collaborating the implementation of each plan of care, coordinating treatment with support staff.
Tracking established clinic metrics for both therapist and administrative staff.
Actively participating in ongoing marketing strategies to ensure the health of our practice.
Willingly participating in community events to help promote our practice.
Be a proactive member of our team bringing proposed solutions to any challenges that may arise.
Foster a professional environment that is friendly and welcoming to everyone.
Display a willingness to go above and beyond to ensure each patient’s satisfaction.
Qualifications Include but are not limited to:
At least 1 year hands on experience as a Licensed DPT.
Ability to handle the physical work load including standing for long periods of time, lifting etc.
Ability to build rapport with patients.
Excellent verbal and written communication skills.
Strong leadership qualities.
Ability to travel locally to patient homes in Nassau and Suffolk Counties.
Covid vaccinated.
Compensation for this position include PTO accrual, and continuing education allowance.
