Wraparound Facilitator for Howard – Indiana Professional Management Group – Kokomo, IN

The Wrap Facilitator works in partnership with emotionally and/or behaviorally challenged children, their families and referral sources to facilitate high-fidelity wrap services, with the ultimate goal of the child remaining safely in the home, school and community. Using a strengths-based, family-driven, culturally competent treatment approach, this position facilitates collaboration between the youth/family and all parties involved in the youth’s treatment. Wrap Facilitation is a dynamic position that requires “out of the box” thinking to develop creative solutions and is often successful for families who have had limited access to traditional outpatient treatment. Wrap Facilitators are directly responsible for providing care coordination with participants and families. This requires extensive time building rapport and partnering with youths/families in wraparound services. Time will primarily be spent gathering the family’s information to develop a family story and identify underlying needs, building effective teams, facilitating monthly team meetings and communicating with the team weekly to monitor participant’s progress. Documentation is also a primary responsibility to maintain fiscal fidelity, and the role requires high attention to detail to ensure all quality measures are met. Wraparound Facilitators will participate in monthly group training and bi-monthly process-based supervision with IPMG Wraparound Supervisor and/or the DMHA Local State Site Coach. Development of community connections and local resources will be required, and this role may encompass outreach and education in order to promote IPMG Wraparound program, as well.

• Facilitate services to Individuals and families (up to 12 Individuals) including facilitation and participation in team meetings
• Maintain weekly contact with Individuals, families and teams

• Maintain and process required documentation accurately and within required timeframes
• Provide weekend, holiday and vacation coverage on-call support on a rotating basis
• Participate in process-based supervision with IPMG Supervisor and/or local DMHA State site Coach twice per month and group supervision monthly
• Participate in IPMG and State required training

• Bachelor’s degree in psychology, counseling, social work, nursing, or other related field
• Minimum 2 years of one or a combination of the following experiences: clinical, case management, skills building, child welfare, juvenile justice, education in a K-12 setting
• Strong analytical skills
• Strong technology skills
• Ability to perform successfully in a virtual environment under minimal direct supervision

• Current and valid driver’s license
• Current and valid car insurance
• Current CPR certification

This position functions in a home office setting, using standard office equipment such as a computer, phone, printer and scanner.

Minimal lifting and reaching is required. This is primarily a sedentary position.

This is a full-time position, with a flexible 40-hour workweek to meet the needs of IPMG, the Individuals and families, with additional hours as needed including weekends and evenings. Weekend, holiday, and vacation coverage “On-Call” hours may be required on a rotating basis.

Significant regional travel is required.
